Where's My Refund-IRS

A quick easy way to check the status of your federal refund via the Internet. You will need to have available your social security number, your filing status (married, single, etc.) and the exact dollar amount of your refund.

Wisconsin "My Tax Account"

State of Wisconsin My Tax Account site for submitting tax forms and making tax liability payments such as state withholding and sales tax. This site is replacing the EFTPS and SIP websites that have been used for submitting sales tax and making tax liability payments.

DOL Form for Employers who Provide Health Insurance coverage

Required form for employers who provide health insurance coverage to employees as part of Section 1512 of the Patient Protection and Affordable Care Act.  This form must be provided to all current employees as of October 1, 2013 and to all newly hired employees within 14 days of employment.

Federal - W-4 Form

Required form for newly hired employees. A copy of this form must be completed by new employees and kept in the company's personnel files.

Wisconsin - WT-4

Required State of Wisconsin form for newly hired employees. A copy of this form must be completed by new employees and forwarded to the State of Wisconsin.

I-9 Form - Employment Eligibility Verification

Required form for newly hired employees. A copy of this form must be completed by new employees and kept in the company's personnel files.

W-9 Form - Request for Identification Number

IRS form used to request verification of a taxpayer identification number and business name. Submit this form to vendors you do business with so all necessary 1099 Forms can be processed at year end.
